ConnectMyHealth is a digital health platform that offers users a single online portal to access their health records from participating hospitals across southwestern Ontario. Accessible on both desktop and mobile devices, it is free to use and does not require an app download. ConnectMyHealth is managed by the HITS eHealth Office at Hamilton Health Sciences, funded by Ontario Health, and is available to patients within the Ontario Health West Region through collaboration with local Ontario Health Teams.
