A valid Ontario health card
An up-to-date mailing address linked to your health card

You can register whether or not you currently have a family doctor or nurse practitioner. If you already have a provider, you may still consider registering — for example, if your provider is retiring or you’ve moved and now live too far away to access their care easily.

If you need medical care before being connected with a family doctor or nurse practitioner:

For emergencies: Call 911 or go to your nearest Emergency Department.
For non-emergency care: For non-emergency care, you can access available healthcare services for Grey Bruce residents by visiting our "Community Resources" web page here.

A Helpful Step Before Your First Doctor or Nurse Practitioner Appointment!

Signing up for ConnectMyHealth is a great way to prepare for your first visit with a new care provider. This free digital tool securely brings together your hospital health records from across the Ontario Health West Region into one place. Having your records easily available can help your new provider quickly understand your medical history and provide the best care.

If you are 16 or older, have a valid Ontario Health Card, and a unique email address, you can register for an account. Parents and Legal Guardians of youth aged 16–17, and Legal Representatives of adults 18+, can also register as Proxy Users to access records on the patient’s behalf. Support is available if you need help signing up.
