This presentation provides an overview of Ontario’s primary care system and the structures, organizations, and initiatives that support the delivery of care across Grey Bruce. It explores key primary care models, funding mechanisms, team-based care approaches, and provincial priorities aimed at improving access and attachment to primary care providers. The presentation also highlights the role of the Grey Bruce Ontario Health Team and Primary Care Network in advancing integrated, patient-centred care and achieving the provincial goal of ensuring every resident has access to comprehensive primary care.
