Primary Care Network

The Grey-Bruce Ontario Health Team (GBOHT) is built on the strong leadership of primary care providers. Their firsthand experience and deep connection to the community shape practical, innovative approaches that address healthcare challenges and strengthen the care delivered across the region. 

What is the GBOHT Primary Care Network?

The Grey-Bruce Ontario Health Team Primary Care Network (PCN) brings together family physicians and nurse practitioners from across the region to strengthen collaboration, share expertise, and shape the future of healthcare in Grey-Bruce.

The PCN serves as the decision-making table for primary care in our OHT. Members meet monthly to identify priorities, address challenges, and co-design solutions that improve care for patients and providers.

Membership

Membership in the Grey-Bruce OHT Primary Care Network is free for Primary Care Providers (Physicians and Nurse Practitioners). It offers the opportunity to partner in the co-design of services within the OHT, collaborate with community partners, and stay informed about programs and resources designed to support practices to provide the best care available for patients in Grey-Bruce.

The Grey-Bruce PCN’s strategic priorities outline the focus areas for strengthening primary care, fostering collaboration, and shaping a healthcare system that meets the unique needs of Grey Bruce.

 

  1. Connect Grey Bruce Primary Care Providers (PCPs) in a way that enables them to serve as a unified voice for primary care at local health system decision making tables.

  2. Connect to Teams and Partners to improve the integration and coordination of care.

  3. Lead local health system delivery and planning that reflects the unique needs and disparities in our region.
