Grey-Bruce Ontario Health Team

The Grey-Bruce Ontario Health Team (Grey-Bruce OHT) is a group of 25+ allied organizations that offers complementary and integrated health and community care to patients in Grey and Bruce counties.

We work together to provide a better healthcare experience.

What is an Ontario Health Team?

Mandated by the Ontario provincial government in 2019, the Ontario Health Team (OHT) model streamlines and integrates services of healthcare providers in a defined geographic region. Members work together to design a local integrated care delivery system that:

  • Addresses LOCAL needs – meets the unique needs of the communities it serves.
  • Provides INTEGRATED healthcare services – improves access to and quality of care and reduces duplication of services.
  • Improves CARE DELIVERY – builds integrated partnerships between all healthcare providers to improve the delivery of services for patients.

What does it mean for patients in Grey and Bruce counties?

Patients access the services they need more easily. They will receive all their care – including primary care, hospital services, mental health and addiction services, long-term care and home and community care from one team.
